Community Care Worker
Main Purpose of Job:
To aid individual customers in their desired wish to remain in and provide access
to, aspects of the local community through undertaking agreed tasks as set out in
each individual care plan.
Responsible to: The Company Partners, Co-ordinator and Senior Staff.
Responsible for: Delivering a service of care to individual customers within operational
Liaison with: Partners, co-ordinator, senior staff, colleagues, customers and where
designated, outside agencies and family members.
Main Tasks of the Job:
To deliver a high standard of care and support to each individual customer in harmony
with policy, procedure and principles laid out by Eve Personal Homecare.
Support, prompt and where necessary undertake self help tasks for customers who are
unable to complete these tasks for themselves, as indicated by the individual care
Monitor the individual customers state of health informing management and appropriate
agencies of any concerns.
Ensure close adherence to the care plan and accurate representation within the running
Maintain required professional standards of care and conduct whilst undertaking tasks
on the customers premises and in the community at large.
Follow all Eve Personal Homecare Policies and Procedures whilst undertaking all tasks,
maintaining Health and Safety at all times.
Encourage customer participation and autonomy of choice in the care that they receive,
aiding the retainment and advancement of life skills.
Respect the rights and wishes of the individual and research both leisure and social
opportunities. Have an enabling attitude.
To communicate effectively in order to maintain the provision of safe consistent
practice, highlighting any occurrences to the appropriate people.
To undertake training deemed appropriate to the developing service and the individual.
