Job Description

 

Job Title:

Community Care Worker

 

Main Purpose of Job:

To aid individual customers in their desired wish to remain in and provide access to, aspects of the local community through undertaking agreed tasks as set out in each individual care plan.

 

Relationships:

  1. Responsible to: The Company Partners, Co-ordinator and Senior Staff.
  2. Responsible for: Delivering a service of care to individual customers within operational guidelines.
  3. Liaison with: Partners, co-ordinator, senior staff, colleagues, customers and where designated, outside agencies and family members.

 

Main Tasks of the Job:
